Recredentialing Checklist for Medical Staff Offices
Recredentialing repeats much of initial credentialing on a schedule, and a checklist helps a small office keep it from slipping.
Know your cycle
Health plans following NCQA standards recredential on a cycle of up to 36 months, and many hospitals reappoint their medical staff on a cycle of two years or less. Your bylaws and contracts set the actual dates.
Each cycle
- Confirm that every license, DEA registration and certification is current.
- Collect updated malpractice insurance and claims history.
- Check for sanctions and exclusions through the sources your policy names.
- Ask the provider to attest to health status, ability to perform the privileges and any changes since the last cycle.
- Review quality and utilization information where the organization requires it.
Track the people, not only the dates
Assign a coordinator to each provider, so that a missing document has an owner. The small practice page describes how a shared workspace can support this.
Document the decision
Keep the verification dates, who performed them and the committee outcome in the file.
Limits of this guide
Rules and fees change, and this guide is general information, so confirm the details with the relevant board or agency before you act. Accreditation standards are revised from time to time.
